Put the following in CAPITALS and bold in your checklist on every single CT brain

I have the following in bold and capital in my checklists during reporting every single CT brain:

  1. Aneurysm - circle of Willis and vertebrobasilar system
  2. Basilar artery thrombosis
  3. Cortical venous sinus thrombosis
Most of the radiologist cast a quick eye in the region of the circle of Willis even in non-contrast CT, but how many actually look along the course of the vertebral artery to look for vertebral artery aneurysm? Vertebral artery aneurysms are are rare, but not diagnosing it on a non-contrast CT led to a death of a young man leaving behind 2 and 4 year daughters. Look at the images as humans, not as scans! There was a case of missed basilar artery thrombosis in 10 day post-partum lady who presented with confusion as the clinical team was considering cortical venous sinus thrombosis and the reporting radiologist was also looking for cortical venous sinus thrombosis on both plain CT and CT venogram. The basilar artery thrombosis was picked up only after 12 hours, potentially missing the best opportunity to treat the lady.
